2. Birth Certificate issued by MCD/Competent Authority.
3. Proof of Resident i.e. Passport/Driving Licence/Ration Card/Election Voting Card(Parents).
4. Latest Photos of Parents with Child.
5. Income Certificate/Affidavit Original of Parents.
6. Photo Copy of Aadhar Card of Parents.
